To register, print and complete the Registration/Sponsorship form and send with a check or credit card information to the Meso Walk/CCCF, 28 W. Market Street, West Chester, PA 19382. Checks may be made payabel to Meso Walk/CCCF. Alternately, you may fax your registration and/or sponsorship form with credit card information to the Chester County Community Foundation at Meso Walk/CCCF, Fax number (610) 696-8213. If you have any fax problems, please call (610) 696-4383. If you choose to use a credit card, please include your NAME as it appears on the card, card Type VISA or MC, Card Number and Expiration Date. Suggested donation is $25 per individual, $50 for family, $200 for organizations.
